What is CS10?

CS10 is a ready-made combination of two research peptides in one vial: the amylin analogue Cagrilintide (5 mg) and the GLP-1 receptor agonist Semaglutide (5 mg).

CS10 combines two separate receptor families. Semaglutide acts on the GLP-1 receptor and cagrilintide on the amylin (AMY1, AMY3) and calcitonin receptors. This sets CS10 apart from single-molecule agonists, such as Tirzepatide or Retatrutide, which combine incretin axes in a single molecule.

Why the combination of two pathways matters

Single molecules — even multi-receptor incretin agonists, such as Tirzepatide — act within the incretin system. CS10 adds an entirely different pathway (amylin), which allows two independent receptor families to be studied in parallel. This makes CS10 a model research tool for studying combined amylin + GLP-1 strategies.

Pharmacokinetics and pharmacodynamics of CS10

Both components of CS10 are acylated peptides with a long half-life thanks to their binding to albumin — with a long half-life (~7 days) for both components.

CS10 vs. Semaglutide vs. Cagrilintide

Clinical studies of CS10

The clinical programme of the cagrilintide + semaglutide combination is developing in two main Phase III studies — REDEFINE 1 (obesity) and REDEFINE 2 (type 2 diabetes). These results come from controlled clinical studies.

Scientific basis — the individual components

Each component of CS10 has its own clinical programme: semaglutide in the STEP (obesity) and SUSTAIN (type 2 diabetes) studies, and cagrilintide in a Phase II dose-finding study in obesity. The combination brings together two independent receptor families in one vial.

REDEFINE 1 — obesity (NEJM, 2025)

In the Phase III REDEFINE 1 study (about 3417 adults with obesity without diabetes, 68 weeks), the cagrilintide + semaglutide combination led to a mean weight reduction of 20.4%, compared with 11.5% with cagrilintide alone, 14.9% with semaglutide alone and 3.0% with placebo. A weight reduction of ≥ 20% was achieved by about 60% of participants and ≥ 30% by about 23%. These results come from a controlled clinical study and are not a recommendation for individual use.

REDEFINE 2 — type 2 diabetes (NEJM, 2025)

In the REDEFINE 2 study (about 1206 adults with type 2 diabetes and overweight/obesity, 68 weeks), the combination led to a weight reduction of −15.7% compared with −3.1% in the placebo group, with a significant improvement in glycaemic control. A weight reduction of ≥ 15% was achieved by 51.6% of participants. These results come from a controlled clinical study.

Research protocols

In the REDEFINE clinical programme the combination was administered in a weekly subcutaneous regimen with gradual dose titration:

Storage and reconstitution

CS10 is supplied as a lyophilised (freeze-dried) powder in a vial containing both components.
